Managing heart failure @home is an innovative approach that aims to support people to better manage their heart failure condition and keep well at home.
Shortlisted for the HSJ Partnership Awards 2025, it uses personalised care, remote support and monitoring and increased integration of primary and secondary clinical services. With support from NHS England, the approach has been tested across ten sites.
A report evaluating the approach, has found that patients experienced an improvement in their health-related quality of life, heart failure related symptoms and ability to manage their heart failure.
